Our connection to a multi-faceted network of community resources allows us to collaboratively find the best solutions that meet your individual needs, improving your service experience and overall clinical outcomes.

SHORT- AND LONG-TERM CARE COORDINATION  SERVICES

Our care coordination services include patient advocacy, care plan oversight, in-person site visits (currently held in accordance with Covid-19 protocols), and care coordination services to ensure that your loved ones are receiving quality health care services in a timely manner, despite your inability to be physically present.

IN-HOME FUNCTIONAL NEEDS ASSESSMENTS FOR THE ELDERLY/DISABLED

This service includes an in-home evaluation to assess a vulnerable adult's ability to maintain their independence within the community. Whether your loved one is in need of a few home health services or placement in a higher level of care, our team's goal is to help them to safely retain as much independence as possible. Our evaluations are provided on-site and are based on the individual's biopsychosocial history, active treatment regimens, and current level of functioning. This service can be bundled with our case management and/or LTC Liaison services.

LONG-TERM CARE (LTC) LIAISON SERVICES

We'll accompany your family on their journey through the world of Adult Family Care Homes (AFCHs), Assisted Living Facilities (ALFs), and Skilled Nursing Facilities (SNFs). We are with you, navigating you through the application and placement process, teaching you what to look for in prospective facilities, and educating you and your family of their rights as a long-term care resident.
